Roads To Independence nursing home transition program is able to assist you or someone you know move from a nursing home to a home in the community. You or your representative must be committed to the process and be willing to advocate for independence. Roads To Independence works with individuals with disabilities and various agencies in transitioning them from nursing homes, assisted living centers, or other institutions into their own homes. This service allows you or someone you know to live more independently in the community. Roads To Independence has two types of nursing home transition programs: those that are eligible for the New Choices Waiver and those that are not, but live in nursing homes, assisted living centers, or other institutions and want to transition to a less restricted environment. The New Choices Waiver is a Medicaid program sponsored by the Utah Department of Health. The program is designed to assist eligible individuals move out of nursing homes into home or community based settings. This program is also available to eligible individuals in assisted living facilities. If you are eligible for the New Choices Waiver, we can assist you or someone you know, move from a nursing home to a home in the community. Supportive services may be provided to you in your home, apartment, a family member’s home, assisted living or other community based setting. Roads To Independence is a Case Management Services provider for the New Choices Waiver! As a case management agency we will:

  • Develop a Comprehensive Assessment and Care Plan based on assessed needs and available resources regardless of the funding source. Update care plans annually and as needed to address changing needs.
  • Assist with accessing Medicaid State Plan services and non-Medicaid resources that may meet your needs.
  • Inform you of your rights to freely choose from the providers and services available to you.
  • Advocate with you to support your goals and services.
  • Monitor service providers to insure that services are being provided according to the assessed need documented in the Comprehensive Care Plan.
  • Obtain support services as needed.
  • Locate affordable and accessible housing as needed.
  • Making your home more accessible as needed.
  • Monitor on an ongoing basis your health and safety status. Initiate appropriate reviews of needs and care plan as indicated.
  • Coordinate services across all Medicaid programs to achieve a holistic approach to care.

  • Independent Living Skills classes
  • Recreational activities
  • Peer mentoring/support
  • Community advocacy
